Format: Author last name, first and middle initial, then 'ed' for editor or
blank if author, book title in italics, edition number, publisher location, publisher,
year of publication, page number(s)

Lexi-Comp, Inc.; 2001:765-8.
Book with multiple authors:
Format: Same format as above, listing the authors as below (if more than 6,
just list the first 3, then "et al")
Example: Lacy CF, Armstrong LL, Goldman MP, Lance LL. Drug Information Handbook, 11th ed. Hudson, Ohio, Lexi-Comp, Inc.; 2003: 1143-7.
Palm software:
Format: Name of company that licensed the software, (name of PDA book
or database). Publisher. Publisher; version and/or date of program accessed.
Example: Lexi-Comp, Inc. (Lexi-Drugs Platinum). Lexi-Comp, Inc.; January 29, 2004.
Databases/online software:
Format: Online database accessed, publisher location: publisher; date database was published, date updated/accessed.
Example: Lexi-Comp Online, Pediatric Lexi-Drugs Online, Hudson, Ohio: Lexi-Comp, Inc.; 2004; January 29, 2004.
